π General Club Forms
- Club Registration Form β For previously existing clubs renewing registration.
- Sample Constitution β Please use this guide for assistance with writing your club constitution.
- New Club Interest Form β For groups interested in starting a new student organization on campus.
- Club Meeting Room Reservation - For club member meetings only. Not to be used for events.
- Club Meeting Minutes β Required for recording meeting notes, votes, and decisions.
π Event Forms
- Event Planning Form β First step in planning your event. Youβll also meet with the Office of Student Affairs & Engagement to finalize and approve your event.
- Event Wrap-up Form - To be completed following club sponsored events.
- Bistro Catering Menu β Review pricing and menu options to support your event planning.
π° Financial Forms
- Club Deposit Form β Required for submitting all cash or check deposits from fundraisers and other activities.
- Credit Card Use Request β For approved in-store purchases.
- Fundraiser/Sale Request β Must be completed for all club fundraisers or sales.
- SGA Funding Request β Submit at least 2 weeks before an SGA meeting to be considered for funding.
- Student Fee Funding Request β For long-term event planning. (Example: Spring 2026 proposals will be accepted in Fall 2025. Announcements will be sent when requests open.)
